Community Learning Disability Teams (18+) CLDTs (incl. Speech & Occupational Therapy)
Service
CLDTs are multidisciplinary teams who support adults with a learning disability and their carers. Specialist health professionals provide support to people with a significant learning disability who, even with reasonable adjustments cannot access generic services and/or require specialist health assessment and treatment. They work closely with social care and other agencies including your GP.
What we do:
- The CLDT can also offer dementia assessments for older people with a learning disability and for people who have Down Syndrome over the age of 30 we can offer screening for early dementia.
- We do not support people who have autism without a significant learning disability or those solely requesting an assessment for autism.
- There are three local community learning disability teams. Contact your nearest team to find out more about the specialist health support that is available and request an assessment.
- Northern CLDT - Crawley, Horsham, Mid Sussex Western CLDT - Chichester, Bognor & Midhurst Coastal CLDT - Shoreham, Worthing, Littlehampton & Lancing
