Chailey Heritage Pathways offers highly specialist care for children, young people and adults with a disability, including physical disabilities and/or sensory impairments and associated health needs.
We are very proud of the warm, caring and professional relationships our staff build with the children, young people and adults, and look forward to widening our reach into the community.
Chailey Heritage Pathways is registered with the Care Quality Commission (CQC).
What we do:
- We provide a flexible way to access highly specialist care and support, both at home and in the community, facilitating access to activities, events or attractions.
- We will provide a flexible community support service whenever and wherever needed. This could include: a few hours a day, all day, weekends, evenings, before or after school and during school holidays.
- Our person-centred packages of care and support are tailored to meet an individuals specific requests, including: shopping trips, social or cultural events, leisure and sport, help with personal care, and nutritional support.
- We work closely with family members to establish individual needs and aspirations.
- At Chailey Heritage Pathways, our support team are subject to a robust training programme, including clinical training delivered by a registered health practitioner.
